BYO Lash Artist Lash Extension Training Consent Form

1. Acknowledgment of Risk and Responsibility

I understand that the application of lash extensions involves various techniques and products that require careful handling and application. I acknowledge that improper application or aftercare can lead to potential risks, including but not limited to:

• Eye infections
• Irritation or allergic reactions
• Damage to natural lashes
• Discomfort or pain
• Temporary or permanent loss of natural lashes
• Vision loss or discomfort

I assume full responsibility for any and all risks associated with the application of lash extensions, whether performed by myself or by a professional.

2. Professional Use and Training Purpose

I understand that this training is intended for professional use and for those seeking to learn the art of lash extension application. This training is not a substitute for professional medical advice, diagnosis, or treatment. I agree to seek further training if I am unable to achieve the desired results or if I encounter any difficulties during the application process.

3. Health and Safety Precautions

I agree to follow all health and safety precautions as outlined in the training materials. This includes, but is not limited to:

• Proper preparation of the eye area
• Daily cleaning and maintenance of lash extensions
• Using only recommended products and tools
• Adhering to the aftercare instructions provided

I understand that failure to follow these guidelines can result in adverse effects, including eye infections, damage to natural lashes, and vision problems.

4. Personal Responsibility and Liability Waiver

I acknowledge that the use of lash extensions and related products is at my own risk. I agree to release and hold harmless Tuani Lash, its owners, employees, and affiliates from any and all claims, damages, or liabilities arising from my participation in this training or the use of any products or techniques demonstrated. This includes any claims for personal injury, property damage, or other losses, whether caused by negligence or otherwise.

5. Informed Consent
